The NHS is slowly expanding access to GLP-1 weight loss medications — but for most people across England, Scotland, Wales and Northern Ireland, waiting lists remain long and eligibility criteria are strict. Here is everything you need to know.
Who Qualifies for NHS GLP-1 Treatment?
As of 2025, NHS access to weight loss medications like Wegovy (semaglutide) is available through two routes:
1. NHS Diabetes Prevention and Treatment Programmes
Those with type 2 diabetes may access semaglutide (Ozempic) through their GP or diabetes specialist. However, Ozempic is licensed for blood sugar control, not primarily weight loss — it remains an off-label use for weight management unless prescribed specifically through a specialist referral.
2. NHS Specialist Weight Management Services (SWMS)
NICE approved Wegovy for use in England in 2023, but access is exclusively through specialist weight management services — a dramatically different pathway from walking into a GP surgery.
Eligibility for SWMS referral typically requires:
- BMI ≥ 35, or BMI ≥ 30 with at least one weight-related comorbidity (type 2 diabetes, hypertension, obstructive sleep apnoea, etc.)
- Evidence that lifestyle interventions have been tried and failed
- Psychological clearance in some services
- No contraindications (pregnancy, pancreatitis history, etc.)
NICE guidance also includes a special provision: people from South Asian, Chinese, Black African or Caribbean backgrounds may qualify at a lower BMI threshold of 27.5, reflecting higher metabolic risk at lower body weight.
What About Scotland, Wales and Northern Ireland?
NHS policy on GLP-1s varies by nation:
- Scotland: SMC has approved Wegovy with similar eligibility criteria to England
- Wales: AWMSG guidance broadly mirrors NICE; access is limited to specialist services
- Northern Ireland: Separate commissioning; access is more restricted and provider-dependent
How Long Is the NHS Waiting List?
In most parts of England, waiting lists for specialist weight management services are 12–36 months. This is the central reason most patients opt for private treatment in the meantime.
Going Private While You Wait
The majority of patients accessing GLP-1 medications in the UK do so privately. Private providers can initiate treatment within 24–48 hours of your online consultation. You pay out of pocket but gain:
- Immediate access (no waiting list)
- All dose strengths without GP approval delays
- Flexibility to adjust or switch medications
- Ongoing clinical support through the app or telehealth
If you are currently on an NHS waiting list, going private in parallel is entirely legal and does not affect your NHS position.
Cities With the Longest Waits
Anecdotal data from UK weight management clinics suggests the longest NHS waits are in:
- London (all boroughs)
- Birmingham (SWMS oversubscribed)
- Manchester and Greater Manchester
- Leeds and West Yorkshire
In these cities especially, private providers offer the fastest route to beginning treatment.
Your Action Plan
- Ask your GP to refer you to your local SWMS — this starts the clock on your NHS waiting list
- While you wait, compare private UK GLP-1 clinics using the GLP-1 Directory
- Choose a GPhC-registered provider with ongoing clinical oversight (not just a one-off prescription delivery service)
Remember: even on a private plan, ensure your GP is kept informed. Some medications interact with existing prescriptions, and your GP needs to be part of your overall healthcare picture.
